UE Prevention Rounds Handout (Pocket Version)

Use for every intubated high-risk patient.

UE Risk Check

1. RASS goal documented and current sedation plan aligned.
2. Restraint status reviewed for appropriateness and integrity.
3. ETT securement method intact and position verified.
4. High-risk flags present: agitation, TBI, prior UE, repeated line/tube pulling.

Readiness Check

1. If SBT ready and passed, plan extubation quickly.
2. If not ready, document risk-mitigation plan for next 12 hours.

If UE Occurs

1. Complete UE event form within 1 hour (RN + RT + provider).
2. Hold bedside huddle within 1 hour.
3. Log event in monthly UE tracker.

Daily Metric to Record

• High-risk airway checklist documented: Yes/No.
